The list of ASHI Accredited laboratories is published by ASHI. Laboratories are listed in alphabetical order by state. Contact information for each laboratory is provided as well as categories in which they are accredited.
Operations Manual
The operation and activities of the Accreditation Review Board are reviewed at several levels. These include ASHI Board of Directors and CMS. Because of the deemed status agreements with CMS, NMDP and UNOS, procedures and policies of the ARB
must be submitted for review and approval. As a result, a detailed Operations Manual was created to provide a single source document for all activities of the ARB

Accreditation Application
Scroll to the bottom of this page to access the accreditation application. Forms are in a PDF format and require the Adobe Acrobat reader. Application Packet Updates
Application packets are all online. When your laboratory cycle comes due, the Accreditation Manager will send your laboratory an email with a link to the application online, a password, and instructions. The information that you enter online will be stored
in a database for years. This information creates a historical record of each laboratory's applications and will serve as the template for future application packets. Future packets will require only changes and updates to the
stored information. Accreditation Cycles
Each laboratory is assigned a cycle which will not change from year to year. Thus, deadlines remain constant within each cycle. New laboratories are assigned to a specific cycle based on when their application is submitted for review.Accreditation fee
The ASHI Accreditation fee is $2,060. Review Board
The first 2 digits of your ASHI laboratory accreditation number indicate what region (non-geographic) you are in. Commissioners are listed in the above link by region. For example: if your ASHI number starts with 12-1-NY-01-1, then your commissioner is
