The list of ASHI Accredited laboratories is published by ASHI. Laboratories are listed in alphabetical order by state. Contact information for each laboratory is provided as well as categories in which they are accredited. Updates and clarifications to this list may be addressed to Melissa (McElroy) Weeks, ASHI Accreditation Manager.
Forms and Documentation
The operation and activities of the Accreditation Review Board are reviewed at several levels. These include ASHI Board of Directors and CMS. Because of the deemed status agreements with CMS, NMDP and UNOS, procedures and policies of the ARB must be submitted for review and approval. As a result a detailed Operations Manual was created to provide a single source document for all activities of the ARB. Questions and clarifications pertaining to this manual may be addressed to Melissa (McElroy) Weeks, ASHI Accreditation Manager.
