Application & Instructions

Application packets are all online.

When your laboratory cycle comes due, the Accreditation Manager will send your laboratory an email with a link to the application online, a password, and instructions. The information that you enter online creates a historical record of your laboratory's applications and will serve as the template for future applications, which will require only changes and updates to the stored information.

Each laboratory is assigned a cycle which will not change from year to year. Thus, deadlines remain constant within each cycle. New laboratories are assigned to a specific cycle based on when their application is submitted for review.

Inspector training requirements and helpful documents are provided to assist the inspector throughout the entire inspection process.
